Clara Catherine Forma 89 was born on November 16, 1935, in the village of Sloan, N.Y., the third child of the late Genevieve & Vincent Januchowski. Clare was the dear sister of her late siblings Vincent Januchowski, Genevieve Bialoblocki and Ronald Januchowski.
Clare was the wife of late John Forma and is survived by her loving sons, John Forma (Tom Marko), Lawrence Forma (Cindy Forma) and Kevin Forma (Matt Randle), granddaughters, Jessica Forma, Sarah Forma and Laura Forma and great grandchildren Damian, Antonio and Aubrey.
Clare was an inspiration and support for many cousins, nieces, nephews and friends. Clare personified love, compassion and the enjoyment of life and will be profoundly missed and loved forever in our memories.

Clare , was full of life . She wanted to enjoy everyday to its fullest . Her sons were her greatest gift . Her friends were so much a part of her life , so many happy times together . As for me , she was a beautiful person , my special cousin, we shared so many good times . I will deeply miss her . We grew up seeing each other almost everyday when we were little . Clara was my godmother. She was always a special person to me and we had a kinship that created a close bond between us as long as I can remember. I have very early memories of her as a child. She was beautiful and vibrant. Such a fun Aunt to have. When I was young I remember her at all the family picnics and holidays. So great to be around. As an adult, Clara was always warm and welcoming to me and my wife through out our lives. After I retired and moved to Florida our relationship grew. Over the last 8 years we have had many laughs and fun times. Hanging out, playing games or going to dinner. She always welcomed me into her home. Clara seemed to constantly go out of her way to make you feel welcome. I will revere our time together drinking margaritas and discussing the Buffalo Bills. I will always love her and cherish our time together. She will be missed.
