Clara Catherine Forma 89 was born on November 16, 1935, in the village of Sloan, N.Y., the third child of the late Genevieve & Vincent Januchowski. Clare was the dear sister of her late siblings Vincent Januchowski, Genevieve Bialoblocki and Ronald Januchowski.

Clare was the wife of late John Forma and is survived by her loving sons, John Forma (Tom Marko), Lawrence Forma (Cindy Forma) and Kevin Forma (Matt Randle), granddaughters, Jessica Forma, Sarah Forma and Laura Forma and great grandchildren Damian, Antonio and Aubrey.

Clare was an inspiration and support for many cousins, nieces, nephews and friends. Clare personified love, compassion and the enjoyment of life and will be profoundly missed and loved forever in our memories.

Clare , was full of life . She wanted to enjoy everyday to its fullest . Her sons were her greatest gift . Her friends were so much a part of her life , so many happy times together . As for me , she was a beautiful person , my special cousin, we shared so many good times . I will deeply miss her . We grew up seeing each other almost everyday when we were little . She opened her arms to everyone . Love you Clare

Clara was my godmother. She was always a special person to me and we had a kinship that created a close bond between us as long as I can remember. I have very early memories of her as a child. She was beautiful and vibrant. Such a fun Aunt to have. When I was young I remember her at all the family picnics and holidays. So great to be around. As an adult, Clara was always warm and welcoming to me and my wife through out our lives. After I retired and moved to Florida our relationship grew. Over the last 8 years we have had many laughs and fun times. Hanging out, playing games or going to dinner. She always welcomed me into her home. Clara seemed to constantly go out of her way to make you feel welcome. I will revere our time together drinking margaritas and discussing the Buffalo Bills. I will always love her and cherish our time together. She will be missed.

Eulogy for Clare Forma

Mom’s Eulogy

I want to thank my Mom for being the third child of 4, whose parents were a loving mother and strict father who died young.

I want to thank my Mom for loving her siblings so much that her 2 oldest siblings continued to visit after Mom moved to Florida until they could no longer make the trip.

I want to thank my Mom for agreeing to meet my Dad at a soda fountain for their first date while in high school, falling in love and marrying him.

I want to thank my Mom and Dad for loving each other and creating 3 children, giving me my life and giving me brothers to love forever.

I want to thank my Mom for being a working Mom, taking time off only briefly for each pregnancy. By doing so, my Mom showed how a woman can be strong, resilient, organized, responsible, and fearless all while raising us in a loving household.

I want to thank my Mom for teaching us how to be balanced, responsible, independent and caring.

I want to thank my Mom for being a great cook who fed us well and left us with memories of our favorite meals.

I want to thank my Mom for being open-minded, challenging herself, accepting differences and providing unconditional love.

I want to thank my Mom for embracing my husband and adopting him as her son.

I want to thank my Mom for expressing interest and compassion to my cousins, many of whom embraced my Mom as their surrogate mother and visited frequently.

I want to thank my Mom for being my advocate, my confidante and my everlasting friend.

I never thought I could live without you, but I learned that I could. I learned that the more love one has for another, the stronger the grief. I also learned that the richness of the experiences one had with one loved so deeply, the stronger the memories and the bigger the joy of having been so fortunate to be so close to this person so loved.

Enjoy your next journey, Mom. Hope you can now embrace Dad, your Mom and all those loved ones who are also on that next journey. Love you forever.
